1. Program Name

CareCoachMD Health Alerts — operated by TrueEval Medical Group.

2. Consent & Opt-In

By providing your mobile phone number to CareCoachMD, you agree to receive autodialed SMS/MMS messages from CareCoachMD at the number provided. Consent is not a condition of purchase of any goods or services. You may opt out at any time (see Section 5 below).

3. Types of Messages

You may receive the following types of text messages from CareCoachMD:

  • Appointment reminders and scheduling confirmations
  • Health and wellness alerts
  • Lab result notifications
  • Medication refill and adherence reminders
  • Daily wellness check-in prompts
  • Two-factor authentication (2FA) codes
  • Account and service notifications

4. Message Frequency

Message frequency varies. Typically 2–10 messages per month depending on your account activity, notification preferences, and enabled features. You can adjust notification preferences in your account settings at any time.

5. Opt-Out Instructions

You can opt out at any time by replying STOP to any message you receive from us.

After you send STOP, you will receive a one-time confirmation message. After that, you will no longer receive SMS messages from CareCoachMD. If you wish to re-subscribe, you may do so through your account settings or by contacting us.

7. Message & Data Rates

Message and data rates may apply.

Your mobile carrier's standard messaging and data rates apply to any messages you send or receive. CareCoachMD does not charge for SMS messages, but your carrier may. Check with your carrier for details about your messaging plan.

8. Carrier Disclaimer

Carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission from your mobile carrier. CareCoachMD is not responsible for messages that are not delivered due to carrier network issues, device incompatibility, or incorrect phone numbers.

9. Supported Carriers

Our SMS program is supported by all major US carriers including AT&T, Verizon, T-Mobile, Sprint, and most regional carriers. Service may not be available on all carrier networks.

10. Privacy & Data Use

Your phone number and SMS data are handled in accordance with our Privacy Policy. We do not sell, rent, or share your phone number or opt-in data with third parties for marketing purposes. Phone numbers collected for SMS are used solely for delivering the message types described above.

11. Terms of Service

Use of CareCoachMD's SMS program is subject to our Terms of Service.

12. Changes to SMS Terms

We may update these SMS Terms & Conditions from time to time. We will notify you of material changes by posting the updated terms on this page and updating the "Last updated" date. Continued use of our SMS program after changes constitutes acceptance of the updated terms.
